383 Madison Project Safety Program
As a contractor on the 383 Madison project, you are required to be enrolled in the JPMorgan Chase Safety Program. In accordance with the enrollment process and JPMC requirements, please complete the following form SafetyEnrollmentForm.pdf and return it to 383madison@esafetysystems.com
Instructions for Completing the Safety Enrollment Form
1. Mandatory Fields
All fields in the Safety Enrollment Form must be completed. As part of this process, you must review and acknowledge all applicable job site policies.
2. Safety Performance Metrics
Provide the following metrics for the past three calendar years:
• EMR
• OSHA Recordable Incident Rate (TRIR)
• Lost Time Rate
These must be reported as rates, not as raw counts of incidents or lost days.
The EMR value is required to be less than or equal to 3.
Formulas:
• Lost Time Rate: [Count of Incidents] x 200.000 / Hours Worked
• OSHA Recordable Rate (TRIR): (Number of OSHA recordable incidents) X 200,000 / (Total number of hours worked)
3. Data Accuracy
Accurate calculations are essential for evaluating safety performance. All rates must be calculated using the formulas provided above.
4. Fatality Records
Report the total number of workplace fatalities for each of the past three years. If no fatalities occurred, enter “0.”
5. Incomplete Fields
If any required field cannot be completed, include a written explanation justifying the omission when submitting the form.
6. Lead Safety
Representative Provide the name and full contact information (email and phone number) of the on‑site Lead Safety Representative responsible for implementing and overseeing safety protocols.
7. Submission Checklist
Before submitting the form, verify that:
• All fields are completed, or a written justification is provided for any blank fields.
• All rate calculations are accurate and follow the required formulas.
• Any supporting documents (e.g., Self‑Insured Status Confirmation Letter) are attached.
THE REGISTRATION PROCESS MUST BE COMPLETED BY A SAFETY MANAGER OR EQUIVALENT FAMILIAR WITH BOTH THE PROJECT SAFETY REQUIREMENTS, AS WELL AS, THE PROJECT SCOPE, EXPOSURES AND SAFETY CONTROLS ANTICIPATED.
